Thigh Liposuction

Thigh liposuction reduces the fatty prominence of the inner thighs, saddlebags, banana rolls under the buttocks, inner knee bulges, and thigh fronts or backs. 

It can be done by itself or combined with other thighplasty procedures. Dr. Agha can also perform it on one or multiple thigh areas, or even the whole thigh.

In fact, it is not uncommon for Dr. Agha to perform large volume circumferential thigh liposuction, removing up to five liters of fat from both thighs from all the four surfaces.

Anterior Thigh Lift

Pioneered by Dr. Agha for his patients, the anterior thigh lift or thigh front lift is a procedure that has not been described in plastic surgery textbooks. 

It uses an incision over the thigh front, through which the thigh skin and fat are loosened up using a specialized instrument. The thigh front is then lifted and anchored to the pubic bone at multiple locations. 

In this manner, the thigh front can be lifted by 1-2 inches, resulting in a smoother thigh contour.

The anterior thigh lift is typically performed as part of a tummy tucklower body lift, or circumferential thigh lift.

Inner Thigh Lift

Also known as the medial thigh lift, the inner thigh lift reduces excess skin and fat in the upper portion of your inner thighs, making it the ideal procedure for people with excess inner thigh skin. 

During a medial thigh lift, Dr. Agha creates an incision at the junction of the thigh and pubic area in your groin crease. He then removes a crescent of excess upper inner thigh skin and fat.

Dr. Agha then lifts the inner thigh all the way to the groin crease at a higher position and secures it to the underlying pubic bone using multiple anchoring sutures. He then reconstructs your groin crease meticulously to prevent it from stretching your labia and flattening your vulva. 

It results in a tighter and smoother inner thigh contour and enhanced skin flow while keeping the scars undetectable under clothing.

Outer Thigh Lift

Also called lateral thigh lift, the outer thigh lift solves excess tissue and skin problems on your outer thighs and saddlebag regions. 

Since the outer or lateral thigh lift is typically performed as part of a lower body lift or combined with a buttock lift or an extended tummy tuck, its incision is essentially an extension of the incisions of these procedures.
Dr. Agha extends the front incision from the lower body lift or extended tummy tuck over the upper outer thigh. He then makes a second parallel incision at a higher position. Through these incisions, Dr. Agha excises the excess tissues of the waist and upper outer thigh. 

He then partially detaches the outer thigh skin and fat from the thigh muscles using a specialized instrument. In this manner, the outer thigh tissue is loosened up.  
The outer thigh tissue is then lifted and anchored back to the thigh muscles at a higher position using multiple anchoring sutures. He closes the incisions in three layers, just like in a tummy tuck or lower body lift.

The outer thigh lift is the most powerful of all thigh lift procedures. Dr. Agha can typically remove 8-12 inches of outer thigh and waist skin for most patients. He has even removed 18 inches in a weight loss patient. The outer thigh lift results in a smoother contour of the outer thigh skin as it flows into the concavity of the waist.

Posterior Thigh Lift

Targeting the back of the thighs, the posterior thigh lift gets rid of loose skin from the banana rolls under the buttocks. It results in a smoother and firmer backside.

Dr. Agha creates an incision in the infragluteal crease that separates the butt from the thigh back. Through this incision, a crescent segment of the upper thigh back under the buttock’s crease is excised to get rid of dimples and banana rolls. 

The thigh back skin is then partially loosened up using specialized instruments. Dr. Agha then lifts the upper thigh and anchors it to the bony prominence of the pubic bone using multiple anchoring sutures. He then closes the incision in three layers.

Spiral Thigh Lift

The spiral thigh lift is a combination of both the inner and posterior thigh lift procedures. 

Pioneered by Drs. Agha and Hurwitz, this thigh lift procedure lifts the back and the inner thigh through an incision that runs along the groin crease and infragluteal crease under the buttocks. Just as its name implies, the spiral thigh lift employs a spiral incision. 

Through this incision, Dr. Agha removes the excess tissue of the upper thigh back, then elevates the thighs to a higher position and secures it to the underlying tissue. He then reconstructs the infragluteal crease, enhancing the shape of both the buttocks and the thighs.

For the laxity on the inner thighs, Dr. Agha continues the incisions over the groin crease. He removes a crescent of upper inner thigh skin, then lifts and secures the inner thighs at a higher position. He also reconstructs the groin crease, resulting in a tighter and smoother thigh contour.

Circumferential Thigh Lift

A circumferential thigh lift is a complex procedure that should only be undertaken by plastic surgeons routinely performing thigh lift surgeries.  

It offers elevation of all the four surfaces of the thighs through an incision that starts in the infragluteal crease under the buttocks, running over the groin crease, and the over the hip towards the back.

The longer incision allows for inner, anterior, outer, and posterior thigh lifts to be performed. When done correctly, the whole thigh is lifted and anchored at a higher position.

Vertical Thighplasty

Also known as thigh reduction, the vertical thighplasty is used for treating the loose skin of the thighs in a circumferential dimension. It is especially useful for treating excess thigh skin that spans from the pubic area to the inner knee on the inner surface of the thighs. 

Through a vertical incision — hence its name of vertical thighplasty — a large ellipse of skin and tissue from the inner thighs is excised from the groin to the inner knees. Dr. Agha then closes the incisions in three layers. The resulting scar will only be seen when the inner thighs are exposed.

Often done on patients who have lost a significant amount of weight, a vertical thighplasty is often combined with other body contouring surgeries such as thigh liposuction.

How Does Dr. Agha Do Thighplasty?


Thigh enhancement is generally considered an outpatient procedure.

More often than not, Dr. Agha combines thigh liposuction with a thighplasty to reduce the appearance of larger thighs. It is also best paired with other body contouring procedures, like lower body lifttummy tuck, and buttock enhancement, for a better overall aesthetic appearance.

Though a thigh lift only takes 2-3 hours to finish, combining it with a lower body lift may take 7-8 hours.

Preparation

During your initial consultation, Dr. Agha will ask you about the following:

  • General physical and mental health status
  • Pre-existing health conditions, especially heart disease, diabetes, thromboembolic disease, connective tissue disorders, and wound healing problems
  • Prior surgeries, including gastric bypass surgery and laparoscopic procedures
  • Medications currently being taken
  • Allergies
  • Smoking habits and alcohol use
  • History of weight gain and loss
  • Psychological issues

Dr. Agha will also discuss your concerns, priorities, and motivations for getting thigh enhancement surgery. He will then give you information on your thighplasty options, then plan an appropriate plan of action that best suits your anatomy and meets your desires and expectations.

Other things that will be discussed include potential risks and complications, estimated cost, and procedure schedule. You will also be asked to obtain specific lab tests and medical evaluation, take or adjust certain medications, and avoid taking aspirin, herbal supplements, and anti-inflammatory drugs that might increase your chances of bleeding.

If you smoke, you will be asked to quit at least three weeks before your thigh lift and remain smoke-free three weeks post-op to avoid any delays in your healing process.

On the day of your surgery, Dr. Agha will mark certain landmarks on your thigh and pelvic bone in the pre-operative area to properly plan the incision and liposuction site. 

You should arrange for a friend or family member to drive you to and from the surgery center, as well as assist you during the first 24 hours post-op.

Anesthesia

A board-certified anesthesiologist will put you under general anesthesia before your thigh lift.

Afterward, Dr. Agha will inject a tumescent solution at the sites of your incision and liposuction. It consists of a low concentration of lidocaine for numbing the surgical site and epinephrine for reducing the potential for bleeding and bruising.

Thigh Liposuction

After injecting tumescent fluid into the marked liposuction areas, Dr. Agha proceeds with liposuction to reduce the fat content of these areas and surrounding tissues.

Dr. Agha inserts a liposuction cannula through one or more small incisions into the deep fat layer of the thighs. He will move the cannula back and forth to break up the fat cells, and then suction it out.

Liposuction can be limited to the inner thighs, banana rolls, saddlebags, or the whole thigh circumference.

Thigh Lift

Dr. Agha creates the incisions of the particular thigh lift procedure and removes the excess upper thigh skin and fat.

After partially detaching the thigh tissue, he lifts the tissue up and anchors it to supporting structures or bone using anchoring sutures. He then closes the incision in three additional layers.

Post-Surgery

Dr. Agha places all his sutures beneath the skin to allow them to be gradually absorbed by the body. Because of this, you no longer have to undergo suture removal, improving your comfort and satisfaction as a patient.

After carefully closing the incisions to minimize your scar, Dr. Agha applies a sterile dressing and wraps a compression garment on the surgical site. The garment helps support your lower body during healing and decreases postoperative swelling and bruising.

Do You Need Thighplasty Revision?

How Do You Recover from Thigh Liposuction and Thigh Lift?

All recovery procedures are focused on minimizing pain and improving your healing. Typically, you will need at least three weeks to heal and resume light activities.

You’ll experience some discomfort and tightness for the first 3-5 days, but these should slowly resolve over time.

If you underwent inner and anterior thigh lift, keep your legs slightly bent at the hips and knees during the first week. This will help reduce pain and tightness in your incision.

You should start walking as soon as possible to avoid the possibility of forming blood clots.

Dr. Agha will recommend a light exercise program that will help reduce swelling and avoid blood clots. Make sure to follow this and avoid exercises that can raise your blood pressure for at least six weeks.

You should also avoid all activities that can put excessive force or stress on your incisions and the operative area until you have fully recovered.

Day of Surgery – Medial thigh lift surgery may be performed as an outpatient procedure, but you may choose to stay in the hospital overnight for a lateral thigh lift with lower body lift. If you choose to go home, you will spend an hour or two in the recovery room prior to being released in the company of a friend or family member. We also have a private duty nurse available to care for you in the comfort and privacy of your own home if you so choose.

Garment – When you wake up from your procedure, you will be in the recovery room and will be wearing a lower body garment. Plan to wear the compression garment for three to four weeks following surgery.

Resting – During the first week after your thigh lift, keep your legs slightly bent at the hips. This ensures that the tension on your incision line is minimized, reducing pain and resulting in a thinner scar during the initial healing process.Mild discomfort and tightness in the thigh area is common for the first 48 to 72 hours. After the third day, the pain will be resolved significantly and the tightness will begin to feel normal.

Dressing – For those who have had an inner thigh lift, it is important to keep the inner thigh and groin crease dry at all times. Place a 4 x 4 gauze on each side and change twice daily.

Painkillers – You’re likely to feel sore for a few days, but you should attempt to get out of the bed in 24 hours. Most of your discomfort can be controlled by the prescribed painkillers. We will assist in the thigh lift recovery time.

On-Q-Pain Pump – Alternatively, you may also be a candidate for a revolutionary new product for the management of post-operative pain, called the On-Q Post-Operative Pain Pump. Be sure to ask Dr. Agha about this during your consultation if you think you might be a good candidate for this system.The inner thigh incisions need daily care to keep them clean and dry. Other dressings can be changed on the third post-operative day. You may now take a shower. After showering, simply pat your incisions dry and replace your abdominal binder.

First Post-op Visit – Dr. Agha prefers to see you on Day 5 for your first post-operative visit. At this time, he will remove the drainage tubes that are used with the lateral thigh lift.

Swelling – Any swelling in your lower body and upper thighs may take 6 to 12 weeks.

Driving – You may drive when driving does not cause pain. This usually occurs in 2-3 weeks. It is not safe to drive a car within twenty-four hours of taking pain medication as your reflexes and alertness may be altered.It is important that the surgical incisions are not subjected to excessive force, abrasion, or motion during the time of healing.

Walking – Even if you find standing difficult during the first post-operative week, it is still important to start walking as soon as possible, usually the evening of surgery. This is the best way to reduce the chance of getting a blood clot in the legs.

Light Exercise – Dr. Agha recommends a program of light exercise following surgery. The motion afforded by this activity will reduce swelling, decrease the possibility of developing blood clots and enhance skin tone.

Activities – During the first week after surgery, you want to avoid activities that raise your blood pressure. This could cause bleeding at the operative site, which could result in a hematoma (collection of blood). Walking is an excellent form of exercise, especially during the first week. Avoid exercises which directly stress the area of your surgery for a full four weeks. After this period, cardiovascular activities, such as riding a stationary bicycle or brisk walking, can be initiated. Avoid heavy lift, contact sports, and jogging for up to six weeks. Six weeks after surgery, you will be nearing your pre-operative state. Avoid stressing the surgical area to maximize healing and minimize scarring. During the early phase of healing, one must avoid irritation around the operative site as this can worsen swelling and fluid accumulation. This could cause the development of a seroma (fluid collection) or cause problems with the wound (dehiscence or opening of the suture line or widening of the scar). Even though the skin is closed, there are many activities taking place beneath the skin.

Healing – You will need a few weeks to recover and heal from a thigh lift procedure. The typical healing time is at least one to three weeks before returning to work.

Scarring – Because of the incisions, there will be scarring. But to make them less noticeable, Dr. Agha hides them in natural skin creases or under a bikini. Every person’s body is different, and many factors contribute to scar healing. Your scars will be firm and pink for about six weeks. Nonsmokers and those who follow the Enhanced Recovery Nutritional Supplements routine have a better chance of having rapid healing and thinner scarring. Expect to wait at least nine months before your scars lighten in color and become as flat as the rest of your skin. Although they will never disappear completely, scars will not show under most clothing.
